Participant Rights

All participants receiving services from Momentum Health Consulting have the right to:

Safety & Quality

  • Receive safe, high-quality clinical care delivered by a qualified Registered Nurse.

  • Access supports that meet professional, ethical and evidence-based standards.

Respect & Dignity

  • Be treated with courtesy, dignity and cultural respect at all times.

  • Have your values, identity, culture, beliefs and preferences acknowledged.

Choice & Control

  • Make informed choices about your supports and how they are delivered.

  • Request a change of service provider or worker at any time.

  • Be actively involved in planning, reviewing and directing your supports.

Privacy & Confidentiality

  • Have your information handled confidentially and stored securely.

  • Decide who your information is shared with.

  • Withdraw consent to share information at any time.

Communication

  • Receive information in a clear, accessible way.

  • Ask questions at any time regarding your care or your plan.

Freedom From Harm

  • Access services free from abuse, neglect, violence, discrimination or exploitation.

  • Report concerns or complaints without fear of negative consequences.

Feedback & Complaints

  • Provide feedback or make a complaint at any time.

  • Have your concerns addressed promptly and respectfully.

  • Escalate complaints to the NDIS Quality and Safeguards Commission if required.

Participant Responsibilities

Participants receiving services from Momentum Health Consulting agree to:

Provide Accurate Information

  • Share relevant information about your health, needs and risks.

  • Inform us of any changes to your health, medications, circumstances or NDIS plan.

Maintain a Safe Environment

  • Ensure the space where services occur is safe and accessible.

  • Notify us if there are hazards that may affect service delivery.

Respectful Interactions

  • Treat staff with courtesy, dignity and respect.

  • Refrain from aggressive, threatening or inappropriate behaviour.

Attend Scheduled Appointments

  • Be available at the agreed time.

  • Provide at least 24 hours’ notice if you need to cancel or reschedule.

Financial Responsibilities

  • Ensure funding is available for services delivered.

  • Pay invoices by their due date (for Self-Managed or private clients).

  • Communicate promptly with your Plan Manager if applicable.

Use Supports Safely

  • Follow clinical guidance provided by your Registered Nurse.

  • Use equipment and health supports as recommended.

Partner in Your Care

  • Participate in assessments and care planning.

  • Ask questions if you do not understand something.

  • Work collaboratively to achieve your NDIS goals.
