What services do you provide?
We provide RN-led clinical assessments, care planning, continence and wound support, clinical training, and high-intensity daily personal activities education. We work with participants, carers, families and support teams.
What areas do you service?
We work across Ballarat and the Central Highlands region, including surrounding towns within approximately one hour of Ballarat. Travel outside this radius may be possible—just ask.
Do you work with all NDIS funding types?
We work with Self-Managed and Plan-Managed participants, as well as privately funded clients. NDIA-managed participants are welcome to use my services, but they would need to pay privately, as only registered providers can invoice the NDIA directly.
How do I make a referral?
Click the “Make a Referral” button on the website. This sends your information directly and is the quickest way for providers, families or support workers to refer.
How soon will I hear back after sending a referral?
You will receive a reply within 1–3 business days to confirm details and schedule the first appointment.
Do you provide training for carers and support workers?
Yes, we offer RN-led practical training sessions including medication administration, continence care, mealtime support, skin integrity, falls prevention and HIDPA skills.
Can you prepare reports for the NDIS?
Yes, we provide clinical summaries, assessment reports and recommendations when required.
Can agencies or support coordinators use your service?
Absolutely, we regularly collaborate with support coordinators, SIL/ILO teams and disability providers to deliver consistent, evidence-based clinical support.v
