What services do you provide?

We provide RN-led clinical assessments, care planning, continence and wound support, and clinical education and guidance. We support participants and support teams with high-intensity daily personal activities through education, guidance, and competency-based support. We work collaboratively with participants, carers, families and support teams.

What areas do you service?

We work across Ballarat and the Central Highlands region, including surrounding towns within approximately one hour of Ballarat. Travel outside this radius may be possible—just ask.

Do you work with all NDIS funding types?

We work with self-managed and plan-managed NDIS participants, as well as privately funded clients. NDIA-managed participants are also welcome to access our services; however, as we are not currently a registered NDIS provider, services would need to be privately paid. Please feel free to get in touch to discuss options.

How do I make a referral?

Click the “Make a Referral” button on the website. This sends your information directly and is the quickest way for providers, families or support workers to refer.

How soon will I hear back after sending a referral?

You will receive a reply within 1–3 business days to confirm details and schedule the first appointment.

Do you provide training for carers and support workers?

Yes, we provide RN-led education, guidance, and practical skill support for carers and support workers. This includes medication support, continence care, mealtime assistance, skin integrity, falls prevention, and high-intensity daily personal activity support.

Can you prepare reports for the NDIS?

Yes, we provide clinical summaries, assessment reports and recommendations when required.

Can agencies or support coordinators use your service?

Yes, we provide clinical summaries, comprehensive assessment reports, and evidence-based recommendations to support NDIS planning, plan reviews, and ongoing care requirements. Reports are tailored to each participant and aligned with NDIS requirements.
