• Training for support workers

HIDPA Worker Training & Competency Assessments
Structured RN-led training to ensure teams can deliver high-risk supports safely and confidently.
Includes:
• PEG feeding
• Catheter care
• Bowel care
• Seizure management
• Diabetes care
• Medication assistance
• Supporting people with high-risk tasks
RN Clinical Reviews & Monitoring
Ongoing clinical oversight to ensure plans remain current, safe and effective.
Includes:
• Monitoring early signs of deterioration
• Reviewing incidents, health changes and concerns
• Updating care plans and strategies
• Clear communication with providers and families
Momentum Health Consulting provides evidence-based clinical assessments, training and care planning - all delivered by an experienced Registered Nurse. We are here to support NDIS participants, families, carers and providers across Ballarat and the Central Highlands region.

We provide clear, transparent pricing for all RN-led clinical services. All fees align with NDIS Pricing Arrangements and industry standards. Below you’ll find a breakdown of service fees, travel charges and funding options. If you’re unsure how your plan covers clinical supports, we’re happy to guide you.
Service Fees
Functional Health & Daily Living Capacity Assessment
Fee: $550
NDIS Category: improved daily living/capacity building.
Includes: comprehensive RN assessment,
functional impact summary,
Recommendations & goals,
clinical report for NDIS planners.
High Intensity Daily Personal Activities (HIDPA) Worker Training & Competency Assessment
Fee: $193.99/hr or $400 fixed-fee for a standard 2-hour + competency session.
NDIS Category: assistance with daily life/capacity building.
Includes: one-on-one or small group training, competency assessment, written documentation for providers.
Health Deterioration/ Escalation Support Plan
Fee: $300–$450
NDIS Category: improved daily living.
Includes: baseline clinical information, early warning signs tailored to the participant, when/how to escalate, GP/ED information, risk mitigation strategies, documentation expectations, written escalation plan.
General Support Worker Clinical Education
Fee: $193.99/hr
Includes: infection prevention, personal care & hygiene, skin integrity basics, understanding clinical risk, recognising deterioration, documentation & reporting.
Travel Fees
Travel is billed in line with NDIS Pricing Arrangements. All travel is calculated from Ballarat CBD.
Service Region
Ballarat & surrounding areas (within approximately 60 minutes of Ballarat CBD)
Travel Time
Travel time is charged at the NDIS-approved hourly rate, capped at 60 minutes each way, as permitted under NDIS rules.
Travel Cost
A travel cost of $1.20 per km applies to cover vehicle and fuel expenses.
This is claimable under the participant’s plan.
Multiple Visits
If travelling to multiple participants in one trip, travel time and kilometres are divided fairly between participants according to NDIS guidelines.
No Travel Fees for Telehealth
Participants accessing telehealth services will not be charged any travel time or travel cost.
Continence, Wound, Catheter & Medication Management Plans
Continence, Wound and Catheter Fee: $500
Wound Assessment + Treatment Plan Fee: $350
Catheter Assessment + Care Plan Fee: $350
Medication Review/Medication Competency Training Fee: $193.99/hr
NDIS Category: capacity building/consumables (where applicable).
Includes: RN assessment, management plan, risk identification, education for carers/supports.
Mealtime Support & Choking Risk Assessment
Fee: $400
NDIS Category: improved daily living.
ncludes: assessment of swallowing risk, identification of choking triggers, texture/consistency recommendations, eating, drinking & positioning advice, safe mealtime strategies for support workers, escalation and emergency response guidance, written mealtime safety plan.
Bowel Health Assessment & Support Plan
Fee: $350
NDIS Category: improved daily living.
Includes: assessment of bowel patterns, constipation risk factors, medication & diet contributors, bowel chart review, PRN protocols, safe bowel support strategies, written bowel health management plan.
Funding Types
Self Managed
You can use your NDIS funds to pay your invoice directly. Invoices are emailed to you and payment is due within 7 days.
Plan Managed
Invoices are sent directly to your plan manager for processing. There are no out-of-pocket costs for participants.
NDIA Managed
At this time, our services are available to self-managed and plan-managed participants only.
NDIA-managed funding requires a registered NDIS provider.
RN Clinical Reviews & Monitoring
Fee: $193.99/hr
NDIS Category: capacity building/improved daily living.
Includes: ongoing clinical oversight. monitoring of health needs, collaborative communication with providers.
Clinical Risk Assessments
Fee: $40
NDIS Category: capacity building.
Includes: environmental risk assessment, falls/choking/mealtime/skin integrity risks, safety recommendations.
Carer & Support Worker Training (RN)
Fee: $193.99
NDIS Category: improved daily living/training.
Topics May Include: high-intensity daily personal activities, mealtime safety & choking preventio, medication administration, skin integrity & pressure care, continence support, infection prevention, using equipment and assistive technology, escalation & recognising deterioration, documentation and incident reporting.
Participants receive: competency assessment (if required), training summary, updated care plan or guidance notes.
