RN-Led Worker Training & Competency Assessment

Participant specific education and competency assessment for support workers delivering clinical and higher risk supports.

Includes:

  • PEG feeding, catheter and bowel care

  • Medication assistance and diabetes care

  • Seizure management and emergency response

  • Infection prevention and hygiene

  • Documentation and reporting skills

  • High Intensity Daily Personal Activity (HIDPA) training and competency assessment

RN Clinical Review, Consultation & Care Coordination

RN clinical support to review changing health needs, coordinate recommendations and help maintain safe and consistent care.

Includes:

  • Review following changes in health or clinical needs

  • Review and updating of clinical support plans

  • Clinical consultation and case discussion

  • Liaison with families, providers and treating health professionals

  • Follow-up after hospital discharge or significant health events

  • Recommendations for ongoing monitoring and escalation

RN Clinical Assessment & Support Planning

Clinical Risk Assessments

Identifying potential risks in daily living, health and the environment to support safer, more effective care planning.

Includes:

  • Risk identification and assessment

  • Falls, skin integrity and clinical risks

  • Strategies to reduce or remove risks

  • Support for funding requests and planning

  • Clear documentation for providers and families

Clinical Education & Workshops

Practical RN-Led education designed to strengthen the clinical knowledge and confidence of disability support teams.

Includes:

  • Medication Safety

  • Recognising and responding to clinical deterioration

  • Infection prevention

  • Wound and pressure injury prevention

  • Documentation and clinical handover

  • Tailored workshops half/full day education sessions.

Fees & Funding

Clear pricing for RN-Led clinical services, NDIS supports and education.

Momentum Health Consulting provides clear and transparent, pricing for RN-Led clinical services. Fees vary depending on the service required, funding arrangement and level of clinical support involved. A quote can be provided before services commence.

Service Fees

This includes assessments, care planning, clinical reviews, consultation, and all associated clinical documentation provided by a Registered Nurse.

Wound, Catheter, Bowel & Medication Support Plans

Individualised RN-developed clinical plans to support safe and consistent care.

May include continence, wound, catheter, medication and other clinical management plans.

Fee: $128.05 per hour for applicable NDIS-funded RN services.

RN Clinical Reviews, Consultation & Care Coordination

One-off or ongoing RN review of health needs, clinical risks, care plans and changes in clinical status.

Fee: $128.05 per hour for applicable NDIS-funded RN services.

Clinical Education & Workshops

Tailored RN-led clinical education for disability support teams and organisations.

Available as: targeted education sessions, half-day workshops and full-day workshops.

Fee: Quote provided on enquiry.

Travel & Service Fees

Fees are charged in accordance with current NDIS Pricing Arrangements and Price Limits.

Travel Time

Travel time may be charged for the time spent travelling to and from a participant when delivering face-to-face services, in line with NDIS Pricing Arrangements.

Travel time is billed at the applicable hourly rate for the service being delivered.

Kilometre / Vehicle Costs

A contribution toward provider travel costs may apply for face-to-face visits.

where applicable, non-labour travel (vehicle costs) is charged in accordance with current NDIS Pricing Arrangements.

Multiple Visits

Where travel is required to see multiple participants in the same area, travel time and kilometre costs may be apportioned between participants in accordance with NDIS guidelines.

Telehealth

Telehealth services do not incur travel time or kilometre charges.

RN Clinical Assessment & Support Planning

Fee: $128.05 per hour for applicable NDIS-funded RN services.

Funding Types

Self-managed

You can use your NDIS funds to pay your invoice directly. Invoices are emailed to you, and payment is due within 7 days.

Plan-managed

Invoices are sent directly to your plan manager for processing. There are no out-of-pocket costs for participants.

NDIA-managed

Currently unavailable. Services are available to self-managed and plan-managed participants only.
NDIA-managed funding requires a registered NDIS provider.

Clinical Risk Assessments

RN assessment of clinical risks with recommendations to support safe care and service delivery.

Fee: $128.05 per hour for applicable NDIS-funded RN services.

RN-Led Worker Training & Competency Assessment

Participant-specific clinical education, practical skills training and competency assessment for support workers.

Fee: Quoted according to participant requirements, training requirements and number of workers.
